Instructions and information on how to enter form part of these terms and conditions, as detailed elsewhere in this document.
To enter the 5Step Incentive Program, participants must be a Pharmacy Choice Silver or Gold level member or healthSAVE® member and must be on StoreIQ. Incentive participants will be required to meet specified incentive program targets to accumulate incentive points. Points will not be retrospectively awarded. If an activity has not been completed or reported by the due date as specified in the points table, points will not be awarded for that activity. The winning pharmacies will be determined by Pharmacy Choice® head office in accordance with these terms and conditions. Winners will be determined as follows: Major prizes will be awarded to 1 Pharmacy of the Year, and the 5 stores that achieve the highest ranking nationally, 2 most improved stores and the 2 best newcomers stores. "Most improved" refers to stores that participated in 2019 5Step Incentive program, have reached a hurdle of 50% of available points in 2020 and have shown the highest point growth from 2019 to 2020. "Best newcomer" refers to a store that has not participated in the 5Step Incentive Program before and has reached a hurdle of 50% of available points. Minor prizes will also be awarded to the next 15 highest ranked stores nationally.
Directors, management and employees of the Promoter and their immediate families and agencies, retailers and suppliers associated with this promotion are ineligible to enter.
All entries become the property of the Promoter. To be valid, all details on the entry form must be completed correctly. All entrants must remain a Pharmacy Choice® Silver or Gold level member or healthSAVE® member for the duration of the incentive period, including the prize.
The Promoter reserves the right to verify the validity of entry and reserves the right to disqualify any entrant for tampering with the entry process or submitting an entry which is not in accordance with these conditions of entry.
Where it reasonably appears to Symbion that the purchasing performance of the participating pharmacy does not satisfy Symbion being used as the participating pharmacy’s primary wholesaler for a continuous period of 60 days, Symbion may at its discretion withdraw the participating pharmacy’s eligibility for any prizes available from the 5Step Incentive Program. In order to meet the definition of primary wholesaler, the participating pharmacy is required to purchase at least 90% of wholesale orders through Symbion, verified through Intellipharm Pty Ltd.
The 2020 5Step Incentive Program commences 12.01am Sunday 1 March 2020 & closes 11.59pm Monday 31 August 2020. Entries to register for the program submitted after 31 March 2020 will not be entitled to enter the incentive program.
The 2020 5Step Incentive Program major prize comprises of: a) Two return economy airfares from the mystery destination, to Sydney, NSW or Melbourne, VIC for an economy domestic airfare to your nearest capital city, upon completion of trip. b) Two tickets, (twin share) for five nights’ accommodation at the destination from Tuesday 27 October until Sunday 1 November 2020, with the return flight landing on 1 November 2020. c) All accommodation and selected meals and daily activities during the prize period.
Winning participants must be ready to travel on these dates. Winning participants will be given 4 weeks' notice.
The 2020 5Step Incentive Program major and minor prize excludes: a) Incidental expenses whilst on trip, such as alcohol, health spa treatments, other daily activities, and any other incidental expenses including, but not limited to, external activities and spending money. b) All expenses associated with you getting to your nearest capital city for domestic departure to prize destination. c) All domestic expenses associated with you returning home from your nearest capital city.
The total value of the prize pool is approximately $200,000.00. The Promoter accepts no responsibility for any variation in the value of prizes.
The prize will be available and delivered only in Australia.
In the event that there are tied results, tied Pharmacies will be required to submit an article for publication in the next available edition of The Pharmacy Choice® Newsletter. One article will be judged as the winner by the promoter for publication, and will win the prize.
The incentive winners will be determined by 16 September 2020 at Level 7, 737 Bourke Street Docklands, Victoria, based on accumulated points gained in the specified criteria as outlined in these terms and conditions. The winners will be notified personally and in writing. The winning pharmacies as determined by Pharmacy Choice® head office will be final and no correspondence will be entered into. Winners must confirm their acceptance of the prize before 20 September 2020. If not accepted within this timeframe, the Promoter reserves the right to award the prize to the next highest ranked pharmacy member.
Prizes are not exchangeable or redeemable for cash. In the event that for any reason a prize-winner does not take an element of the prize, then that element of the prize will be forfeited by the prize winner and cash will not be awarded in lieu of that prize. The recipient may transfer the prize to another staff member at his/her discretion if written notice of such transfer is provided to the Promoter before 20 September 2020.
Prizes won in Spin To Win Activity are limited to 2 prices per store.
If the winners are under the age of 18 years, the prize will be awarded to the winner's parent or guardian.

No responsibility accepted for late, lost or misdirected entries.
The Promoter accepts no responsibility of any tax implications that may arise from the prize winnings. The winner should seek independent financial advice.
The Promoter will not be liable for any loss or damage whatsoever which is suffered (including but not limited to indirect or consequential loss) or for any personal injuries suffered or sustained relating to the prize except for any liability which cannot be excluded by law. The Prize does not include travel insurance, passports, visas or spending money. The Promoter strongly recommends that the winner and any travelling companions purchase travel insurance. Compliance with any health or other government requirements is the responsibility of the prize-winner and his/her nominated travel companion. Prize travel is subject to fare rules and the General Conditions of Carriage of the relevant carrier. The Promoter makes no representation as to the safety, conditions or other issues that may exist at any destination.
If for any reason, this promotion is not capable of running as planned, including war, terrorism, state of emergency or disaster, tampering, unauthorised intervention, fraud, technical failures or any other causes beyond the control of the Promoter which corrupt or affect the administration, security, fairness, integrity or proper conduct of this competition, the Promoter reserves the right in its sole discretion to cancel, terminate, modify or suspend the competition.
Privacy statement. The personal information that you provide to enter this competition ("Information") will be used by the Promoter and its marketing and promotional agencies to conduct this competition. If you do not provide the Information, we will not be able to enter you in the competition. If you do not truthfully provide the Information, the Promoter may determine that you are ineligible to win the prize. Winner's name will be published and retained as required under the applicable legislation. You can find out how we deal with personal information and access the Promoter's full privacy policy by accessing the Promoter's website at www.symbion.com.au or contacting the Promoter at the address above. Any request to access or update personal information should be made to the Promoter at the address listed above. The Promoter will generally provide you with access to your information if practicable (although a reasonable fee may be imposed).
The Promoter reserves the right to publicise winners (including name, address and business) throughout broadcast, electronic and print media, point of sale and newsletters, without compensation.
